We created this workbook to pull in
1) Users
2) Transaction activity and inactivity
3) Query activity and inactivity
4) Workflow activity and inactivity
This was for Foundation v12. I'm attaching a version. You will need to update the Data Connection Queries - updating your Workflow DB name
Click to edit the query
then edit the source
then edit the workflow db names

Note that you will need access to the Licensing DB, which had extra security.
Attached is the Excel workbook example, as well as a Word doc with the SQL queries.

We are working toward migrating from Foundation 12.1 to Evolve. One of the things we'd like to have from Foundation is a usage report. We need one that tells us what users have used the forms in Foundation. Not just who is an approver and ran web services or transactions, but who has been in any step of the workflow. Does anyone know if a report like that exists or can be somehow produced in Foundation? I don't know if there is one in Evolve, but can imagine we'd like one there too.
