The business key designation is only in an import, IF you select the option to Use Business Key. Then if it encounters duplicates, they will be flagged. If all records must have unique business keys, then a Bulk Callout validation rule should be defined on them, flagging any duplicates.

Subject: Using Business Keys Which Are Not PK
I have a scenario in a profile where the "Is Business Key" is checked and "Business Key No Duplicate Option: No duplicates" is selected. However, this attribute is not part of the primary key. The PK for this repo is a auto generated ID defined as BIGINT.
I was expecting some notification/signal if duplication among Business Key exists. However, this is not the case as there is no signal by the system for this.
